A psychiatric patient has drowned in a Bradford reservoir, despite desperate efforts by his carer to save him.
James Tippetts, 57, an in-patient at Lynfield Mount Hospital, got into difficulties after plunging into the water at Chellow Dene reservoir, not far from the hospital, at Chellow Dene.
The patient was walking with his carer on the bank of the reservoir during a routine supervised visit when he went into the water.
After swimming about 50ft out he got into difficulties in the cold water.
His carer swam out to him and managed to drag him back to the bank and tried to resuscitate him.
Paramedics then continued revival attempts for a further 25 minutes but the man was pronounced dead at the scene on Saturday afternoon.
Bradford District Care Trust, which runs the hospital, has started a full investigation into the incident.
John Keaveny, the duty manager at Lynfield Mount, confirmed the inpatient, who has not yet been named, was on a supervised visit to the reservoir when he drowned despite the attempts of the male carer to rescue him.
Mr Keaveny said: "There will be a full investigation by senior managers within the Bradford District Care Trust put into place to investigate this matter. It may also include external investigators but it is too early to say yet.
"We deeply regret the incident and it has been a tremendous shock to all the staff and persons involved."
West Yorkshire Ambulance Service was alerted at 2.25pm and a crew from the Bradford station were on the scene six minutes later. The air ambulance also attended but was not needed.
John Kane, operational supervisor for WYMAS in Bradford, said they had spent 20 to 25 minutes trying to resuscitate the man but he was pronounced dead at the scene.
He said: "Despite using all our resources, including four paramedics and an emergency medical technician, unfortunately we were unable to save the gentleman."
Detective Inspector Gerry O'Shea, of Bradford North CID, said the man's death was not being treated as suspicious and he praised the "brave" rescue attempt by the carer.
He said: "At 2.25pm on Saturday a 57-year-old man was discovered in the water at Chellow Dene reservoir. The man, a patient at Lynfield Mount Hospital, was on a supervised outing from the hospital.
"He was removed from the water in a brave rescue attempt by his carer but unfortunately was pronounced dead at the scene. His carer jumped in the water and managed to bring the man to the bank of the reservoir where he tried to resuscitate him.
"There are no suspicious circumstances and the death has been reported to the coroner."
