A BRADFORD care home has been placed in special measures following an inspection by the Care Quality Commission (CQC).
The watchdog rated Bierley Court on Bierley Lane, Bierley, 'inadequate', though a spokesperson for the home said the process to challenge elements of the report was underway.
The inspection of the home, which is run by Handsale Limited, was carried out on two dates in November last year.
It was rated 'inadequate' in the safe, effective, responsive and well-led categories.
It was judged to be 'requires improvement' in the category of caring.
In the report, inspectors said: "People were not always safe. People were at risk of harm as the provider had not identified, assessed or mitigated risks.
"This included risks related to people's health and care needs as well as environmental risks.
"Some areas of the home were not clean and infection control was not well managed."
It added: "Environmental risks had not been managed safely.
"On three occasions people and staff had been stuck in the lift and twice had to be rescued by the fire brigade.
"The lift was still being used even though it was known to be faulty and could entrap people again."
Concerns were also raised about two people who "were regularly sleeping in each other's bedrooms".
The report said: "Staff said they left them to it as both people were confused about which were their rooms."
Some positives were mentioned in the report, as inspectors said: "People and relatives were generally positive about the service. Staff were described as kind and caring."
They added: "The provider took action during the inspection to address some of the issues we raised."
A spokesperson for Handsale Limited said they were disappointed with the findings.
They claimed the report contained inaccuracies and a formal process to challenge those is underway.
The spokesperson also claimed positive feedback and evidence were not taken into consideration "especially from external professionals around their findings regarding the lift and infection control".
They added: "However, we acknowledge that there were areas of improvement at the home and are glad to see these are being made at a rapid pace with support and oversight from our partners at the local authority and NHS colleagues.
"We would also like to express our thanks for the support of the residents and their loved ones during this time.”
