Patients in Bradford and Airedale who have suffered a heart attack or stroke will be helped to learn more about their condition.
The district has a higher-than-national-average number of deaths from cardiovascular disease, and emergency hospital admission for stroke and coronary heart disease are among the highest in the country.
A new project aims to encourage GPs and other health professionals to improve treatment and management of patients.
Unveiled at a conference in Bradford, the project has the support of the three local NHS clinical commissioning groups – Airedale, Wharfe-dale and Craven, Bradford City, and Bradford Districts.
There is already a range of support available to patients but the new system will make sure patients receive the best possible care, medication and lifestyle advice.
